Advancing integrated care for obesity and cardiometabolic diseases

IMPACT-MED is a five-year, €22.9 million project supported by the Innovative Health Initiative Joint Undertaking. The consortium brings together 26 partners across 13 countries to develop a more integrated, multidisciplinary and person-centred approach to the prevention, diagnosis and long-term management of cardiometabolic diseases, including obesity, diabetes, hypertension and cardiovascular disease.

The project will develop coordinated care pathways supported by digital technologies, remote monitoring, advanced diagnostics and patient-reported information. Four clinical studies across Europe will test the approach in real healthcare settings, including personalised care for people living with type 2 diabetes and obesity.
