Conform\u00e9ment au cadre EASO publi\u00e9 dans Nature Medicine, <a href=\"https:\/\/easo.org\/fr\/a-new-framework-for-the-diagnosis-staging-and-management-of-obesity-in-adults\/\">https:\/\/easo.org\/a-new-framework-for-the-diagnosis-staging-and-management-of-obesity-in-adults\/<\/a> La d\u00e9claration recommande vivement d'utiliser des mesures anthropom\u00e9triques suppl\u00e9mentaires, telles que le rapport taille-taille (WHtR), et pr\u00e9conise l'utilisation d'outils de diagnostic avanc\u00e9s, tels que l'analyse vectorielle de l'imp\u00e9dance bio\u00e9lectrique (BIVA).<\/p>\n<p>Les points saillants de la d\u00e9claration sont les suivants :<\/p>\n<ul>\n<li>Fertilit\u00e9 et SOPK : L'ob\u00e9sit\u00e9 peut nuire \u00e0 la fertilit\u00e9, m\u00eame chez les femmes ayant des cycles r\u00e9guliers. Une perte de poids mod\u00e9r\u00e9e (5 \u00e0 10 kg) sur six mois peut r\u00e9tablir l'ovulation. Les femmes atteintes du SOPK peuvent tirer profit d'un changement de mode de vie, de la metformine ou des agonistes des r\u00e9cepteurs du GLP-1.<\/li>\n<li>Soins pr\u00e9nataux : optimiser son poids avant la grossesse am\u00e9liore les r\u00e9sultats. La d\u00e9claration pr\u00e9conise un d\u00e9pistage syst\u00e9matique des comorbidit\u00e9s, une suppl\u00e9mentation en folates et une contraception pour les femmes qui suivent un traitement pharmacologique ou chirurgical contre l'ob\u00e9sit\u00e9.<\/li>\n<li>Conseils pendant la grossesse : il est essentiel de fournir des recommandations personnalis\u00e9es concernant la prise de poids pendant la grossesse (GWG). L'EASO recommande une prise de poids comprise entre 5 et 9 kg pour les femmes souffrant d'ob\u00e9sit\u00e9 avant la grossesse et encourage les interventions en mati\u00e8re d'alimentation et d'activit\u00e9 physique.<\/li>\n<li>S\u00e9curit\u00e9 pendant la grossesse : les femmes ob\u00e8ses courent un risque plus \u00e9lev\u00e9 de diab\u00e8te gestationnel (DG), de pr\u00e9\u00e9clampsie et d'anomalies f\u0153tales. La d\u00e9claration pr\u00e9conise un d\u00e9pistage personnalis\u00e9 et une surveillance f\u0153tale accrue, en particulier au cours du troisi\u00e8me trimestre.<\/li>\n<li>Soutien post-partum et allaitement : l'EASO recommande vivement la mise en place d'un accompagnement structur\u00e9 en mati\u00e8re de poids post-partum, de planification contraceptive et de soutien \u00e0 l'allaitement, afin d'aider les femmes ob\u00e8ses \u00e0 commencer et \u00e0 poursuivre l'allaitement.<\/li>\n<li>M\u00e9dicaments et chirurgie : Le document souligne le manque de donn\u00e9es sur la s\u00e9curit\u00e9 des m\u00e9dicaments contre l'ob\u00e9sit\u00e9 pendant la grossesse et l'allaitement. La chirurgie bariatrique m\u00e9tabolique peut \u00eatre appropri\u00e9e dans certains cas, avec un d\u00e9lai minimum de 12 \u00e0 18 mois avant la conception.<\/li>\n<\/ul>\n<p>En comblant les lacunes en mati\u00e8re d'orientation et en pr\u00f4nant une approche centr\u00e9e sur la personne et fond\u00e9e sur des donn\u00e9es probantes, l'EASO souligne la n\u00e9cessit\u00e9 d'offrir des soins adapt\u00e9s et un soutien continu aux femmes souffrant d'ob\u00e9sit\u00e9 tout au long de leur vie reproductive.<\/p>\n<p>Le Dr Andreea Ciudin, copr\u00e9sidente du groupe de travail sur la gestion de l'ob\u00e9sit\u00e9 de l'EASO et auteure correspondante de la d\u00e9claration (H\u00f4pital universitaire Vall Hebron, Barcelone) a d\u00e9clar\u00e9<\/p>\n<blockquote><p><em>\u201c Les femmes souffrant d'ob\u00e9sit\u00e9 sont confront\u00e9es \u00e0 des probl\u00e8mes de sant\u00e9 particuliers tout au long de leur vie reproductive.
