Les \u00e9tudes sur les facteurs environnementaux, professionnels et m\u00eame li\u00e9s au d\u00e9but de la vie accordent une attention croissante aux facteurs de risque de la BPCO autres que le tabagisme. Bien que des \u00e9tudes ant\u00e9rieures aient sugg\u00e9r\u00e9 un lien entre l'adiposit\u00e9, l'asthme non allergique et la fonction pulmonaire chez les adultes, l'association avec la BPCO reste \u00e0 \u00e9lucider. L'objectif \u00e9tait donc d'examiner si les trajectoires de l'indice de masse corporelle (IMC) pendant l'enfance \u00e9taient associ\u00e9es \u00e0 la BPCO.<\/p>\n

Les auteurs ont inclus les donn\u00e9es de 276 747 enfants danois (dont 137 493 filles) n\u00e9s entre 1930 et 1982, qui avaient fait l'objet de 2 \u00e0 12 mesures de poids et de taille entre 6 et 15 ans, issues du registre des dossiers m\u00e9dicaux scolaires de Copenhague. \u00c0 l'aide d'un mod\u00e8le statistique, cinq trajectoires distinctes de l'IMC chez les enfants ont \u00e9t\u00e9 identifi\u00e9es : inf\u00e9rieur \u00e0 la moyenne, moyen, sup\u00e9rieur \u00e0 la moyenne, surpoids et ob\u00e9sit\u00e9.<\/p>\n

Les auteurs ont ensuite suivi ces personnes de 1977 \u00e0 2022 dans les registres nationaux de sant\u00e9 et ont identifi\u00e9 celles qui avaient re\u00e7u un diagnostic de BPCO \u00e0 partir de 40 ans. Les rapports de risque (HR) pour la BPCO ont \u00e9t\u00e9 estim\u00e9s s\u00e9par\u00e9ment pour les femmes et les hommes \u00e0 l'aide d'une mod\u00e9lisation statistique.<\/p>\n

Les auteurs ont constat\u00e9 que, pendant la p\u00e9riode de suivi, 18 227 femmes et 15 789 hommes ont re\u00e7u un diagnostic de MPOC. Par rapport aux femmes ayant une trajectoire moyenne de l'IMC pendant l'enfance, les risques de MPOC chronique \u00e9taient 10% plus \u00e9lev\u00e9s chez les femmes ayant une trajectoire sup\u00e9rieure \u00e0 la moyenne, 26% plus \u00e9lev\u00e9s chez les femmes ayant une trajectoire de surpoids et 65% plus \u00e9lev\u00e9s chez les femmes ayant une trajectoire d'ob\u00e9sit\u00e9.<\/p>\n

Par rapport aux hommes ayant une trajectoire moyenne de l'IMC pendant l'enfance, les risques de MPOC \u00e9taient 7% plus \u00e9lev\u00e9s chez les hommes ayant une trajectoire sup\u00e9rieure \u00e0 la moyenne, 16% plus \u00e9lev\u00e9s chez les hommes ayant une trajectoire de surpoids et 40% plus \u00e9lev\u00e9s chez les hommes ayant une trajectoire d'ob\u00e9sit\u00e9.<\/p>\n

En revanche, un risque moindre de d\u00e9velopper une BPCO \u00e0 l'avenir n'a \u00e9t\u00e9 observ\u00e9 que chez les femmes dont l'\u00e9volution de l'IMC pendant l'enfance \u00e9tait inf\u00e9rieure \u00e0 la moyenne, soit 9% inf\u00e9rieure \u00e0 celle des femmes dont l'\u00e9volution de l'IMC pendant l'enfance \u00e9tait moyenne.<\/p>\n

Les auteurs concluent : \u201c Avoir un IMC sup\u00e9rieur \u00e0 la moyenne pendant l'enfance peut augmenter le risque de d\u00e9velopper une BPCO par la suite. Ainsi, nos r\u00e9sultats sugg\u00e8rent que le surpoids pendant cette p\u00e9riode pr\u00e9coce de la vie est un indicateur du risque de d\u00e9velopper une BPCO. \u201d<\/p>\n

R\u00e9fl\u00e9chissant aux causes potentielles de cette association, ils ajoutent : \u201c Le tabagisme parental et le statut socio-\u00e9conomique sont des facteurs de confusion potentiels et non mesur\u00e9s dans cette \u00e9tude. Cependant, \u00e9tant donn\u00e9 que les estimations des effets \u00e9taient coh\u00e9rentes entre les cohortes de naissance, malgr\u00e9 divers changements m\u00e9dicaux, culturels et socio-\u00e9conomiques (y compris les habitudes tabagiques) au cours de la p\u00e9riode \u00e9tudi\u00e9e, le biais est consid\u00e9r\u00e9 comme limit\u00e9, ce qui signifie que le surpoids et l'ob\u00e9sit\u00e9 pendant l'enfance sont probablement des indicateurs ind\u00e9pendants du risque de BPCO \u00e0 l'\u00e2ge adulte. \u201d<\/p>\n
