Compte tenu des diff\u00e9rentes approches adopt\u00e9es aux \u00c9tats-Unis, au Canada et dans les pays europ\u00e9ens en mati\u00e8re de prise en charge de l'ob\u00e9sit\u00e9 infantile, quels ont \u00e9t\u00e9 les principaux d\u00e9fis \u00e0 relever pour parvenir \u00e0 un consensus entre les diff\u00e9rents acteurs concern\u00e9s sur les crit\u00e8res d'\u00e9valuation primaires et secondaires \u00e0 mesurer dans les essais cliniques sur les traitements pharmacologiques de l'ob\u00e9sit\u00e9 infantile ?<\/em><\/p>\n<p>\u2013 Notre plus grand d\u00e9fi consistait \u00e0 trouver un crit\u00e8re d'\u00e9valuation qui refl\u00e8te fid\u00e8lement la gravit\u00e9 et la pathologie de l'ob\u00e9sit\u00e9, qui indique d'autres r\u00e9sultats en mati\u00e8re de sant\u00e9 et de qualit\u00e9 de vie, et surtout qui ne soit pas contraignant et facile \u00e0 comprendre pour les patients et leurs familles, les chercheurs et les professionnels de sant\u00e9.<\/p>\n<p><em>Je suis s\u00fbr que nos lecteurs souhaitent comprendre pourquoi le groupe a d\u00e9cid\u00e9 de ne plus utiliser le score z de l'IMC comme crit\u00e8re principal et quelles autres m\u00e9thodes ont \u00e9t\u00e9 envisag\u00e9es pour mesurer le succ\u00e8s de ces essais.<\/em><\/p>\n<p>\u2013 Au niveau international, il existe des diff\u00e9rences importantes dans la mani\u00e8re dont l'ob\u00e9sit\u00e9, et en particulier l'ob\u00e9sit\u00e9 s\u00e9v\u00e8re, est class\u00e9e chez les enfants et les adolescents dans les r\u00e9f\u00e9rences. Il en r\u00e9sulte des diff\u00e9rences dans la signification des scores z de l'IMC en termes de variation de l'IMC, ce qui complique l'interpr\u00e9tation du traitement. Un crit\u00e8re d'\u00e9valuation id\u00e9al est celui qui quantifie avec pr\u00e9cision la quantit\u00e9 et la r\u00e9partition de l'adiposit\u00e9, mais ceux-ci ne sont pas facilement disponibles ou valid\u00e9s. D'ici l\u00e0, nous recommandons vivement d'utiliser d'autres mesures de l'IMC, telles que le pourcentage du 95e percentile ou le pourcentage du percentile m\u00e9dian, comme r\u00e9sultat principal. Il est \u00e9galement important de noter que nous recommandons de rapporter plusieurs mesures de l'IMC afin de faciliter les comparaisons entre les \u00e9tudes.<\/p>\n<p><em>Le nouvel article traite des pr\u00e9occupations \u00e9thiques li\u00e9es aux longues p\u00e9riodes de suivi apr\u00e8s l'arr\u00eat du traitement, en raison du risque de reprise de poids chez les enfants. Comment l'\u00e9quipe a-t-elle concili\u00e9 ces pr\u00e9occupations \u00e9thiques avec la n\u00e9cessit\u00e9 de comprendre les effets \u00e0 long terme du traitement ?<\/em><\/p>\n<p>\u2013 Nous sugg\u00e9rons de mettre fin aux longues phases de suivi, car elles peuvent limiter l'acc\u00e8s des participants \u00e0 des traitements potentiellement efficaces.
