Ce nouveau cadre, publi\u00e9 dans Nature Medicine, s'appuie sur les r\u00e9sultats d'une \u00e9tude Delphi modifi\u00e9e men\u00e9e par l'EASO, \u00e0 laquelle ont particip\u00e9 de nombreux leaders d'opinion cl\u00e9s dans le domaine de l'ob\u00e9sit\u00e9. Les r\u00e9sultats de cette \u00e9tude, dirig\u00e9e par le Dr Luca Busetto (Universit\u00e9 de Padoue, Italie) et le professeur Gijs Goossens (Universit\u00e9 de Maastricht, Pays-Bas), ont \u00e9t\u00e9 publi\u00e9s aujourd'hui dans Nature Medicine. <a href=\"https:\/\/doi.org\/10.1038\/s41591-024-03095-3\" target=\"_blank\" rel=\"noopener\">https:\/\/doi.org\/10.1038\/s41591-024-03095-3<\/a> <a href=\"https:\/\/www.nature.com\/articles\/s41591-024-03095-3\" target=\"_blank\" rel=\"noopener\">https:\/\/www.nature.com\/articles\/s41591-024-03095-3<\/a><\/p>\n<p>L'ob\u00e9sit\u00e9 est une maladie multifactorielle, chronique, r\u00e9cidivante et non transmissible, caract\u00e9ris\u00e9e par une accumulation anormale et\/ou excessive de graisse corporelle qui pr\u00e9sente un risque pour la sant\u00e9. Il est bien \u00e9tabli que l'ob\u00e9sit\u00e9 favorise l'apparition d'une s\u00e9rie d'autres maladies non transmissibles et transmissibles. Malgr\u00e9 cette large reconnaissance de l'ob\u00e9sit\u00e9 en tant que maladie chronique, les recommandations cliniques qui guident le diagnostic de l'ob\u00e9sit\u00e9 et sa prise en charge n'ont pas \u00e9t\u00e9 suffisamment align\u00e9es sur les processus cliniques normalement adopt\u00e9s pour d'autres maladies chroniques. Dans de nombreux contextes, le diagnostic de l'ob\u00e9sit\u00e9 repose encore uniquement sur les valeurs seuils de l'indice de masse corporelle (IMC) et ne refl\u00e8te pas le r\u00f4le de la r\u00e9partition et de la fonction des tissus adipeux dans la gravit\u00e9 de la maladie. L'accumulation de graisse abdominale est associ\u00e9e \u00e0 un risque accru de complications cardiom\u00e9taboliques et constitue un facteur d\u00e9terminant plus important que l'IMC dans le d\u00e9veloppement de la maladie, m\u00eame chez les personnes dont l'IMC est inf\u00e9rieur aux valeurs seuils standard pour le diagnostic de l'ob\u00e9sit\u00e9. Ce dernier point est soulign\u00e9 dans ce nouveau cadre, car il est important de r\u00e9duire le risque de sous-traitement dans ce dernier groupe de patients, selon les auteurs de l'article. De plus, les indications pour l'utilisation des diff\u00e9rentes approches th\u00e9rapeutiques actuellement disponibles pour la prise en charge de l'ob\u00e9sit\u00e9 restent \u00e9galement principalement bas\u00e9es sur des mesures anthropom\u00e9triques, plut\u00f4t que sur une \u00e9valuation clinique plus compl\u00e8te du patient, tenant compte des troubles m\u00e9dicaux, fonctionnels et psychologiques.
